The Apopka Health Clinic is an interpersonal, interprofessional, service learning opportunity for students from UCF School of Social Work, Doctor of Physical Therapy Program, College of Medicine and College of Nursing, along with pharmacy students from the University of Florida to provide health care to underinsured and uninsured patients during a free health clinic in Apopka, Florida.
The ASSURE model was used to analyze, design, and develop a new training course for students participating in the clinic.
To begin, I conducted face-to-face and phone interviews with subject matter experts including Apopka Clinic faculty directors and the UCF College of Medicine Medical Students Providing Across Continents (MedPACt) leaders. Given time constraints and identified needs for this project, I used the ASSURE model to design and develop the learning environment and EMR training. I analyzed the learners, stated objectives, selected methods, media, and materials, utilized media and materials, and required learner participation. Based on feedback and observations, course materials will be evaluated and revised for future clinics.
Identified Constraints
Constraints include accelerated schedule, limited access to SMEs, and identifying skills, knowledge, and resources applicable to all students (interprofessional development). Motivation also varies among students (e.g., MD students volunteer, but nursing students are required to attend and participate). Also, MD students are at various stages in their program, while students from other colleges are ready to graduate. Finally, every role/station has different responsibilities, but the underlying theme is teamwork and patient care.
After meeting with UCF faculty and UCF MD students and discussing their needs, I created a performance support site for students and faculty in the free version of Canvas by Instructure. This site provides students and faculty access to supplemental training and resources specific to the Apopka Clinic. Canvas was selected for this site because students and faculty are familiar with this LMS for their daily coursework (i.e., Webcourses@UCF). The free version also allows access for volunteers and UF faculty and students. For the scope of this internship, I created and published three modules (Overview, EMR training, and Medical Spanish) and a "Know before you go" five question formative quiz. After the site was created and approved, 84 people were enrolled (77 students, 4 TAs, and 3 teachers) one week prior to the face-to-face training session. In addition to creating the site in Canvas, I also created the UCF Comprehensive Medical Care Outreach Team (COMCOT) logo and banner for the site and training materials.
Identified Constraints
UCF students would eventually like to access the course through Webcourses, but using the free version of Canvas allows site admins to provide access to UF Pharmacy students and faculty, as well as community volunteers. This course/site is specific to the Apopka Clinic and assumes students have the prerequisite knowledge and entry skills related to their discipline.
A self-learning module (SLM) was created in Articulate Storyline 360 for UCF MD students to review the electronic medical records (EMR) system prior to attending face-to-face training and clinic. Camtasia was used to screen record the EMR and edit the videos prior to importing them into Articulate. The images used in the SLM were photographs taken at previous Apopka clinics. The SLM is organized by stations/roles at the clinic. Two of the modules have formative review questions (assessments) and one of the modules has a “try-it” activity that simulates actions in the EMR.
After the Apopka clinic on June 28, 2018, students and faculty were asked to complete an electronic survey (Google form) regarding various aspects of the clinic and training prior to clinic. Of the 55 questions in the student survey, 10 were related to training:
Have you had prior experience with electronic health systems?
How was your experience using the Apopka EMR?
How helpful was the EMR training session?
The EMR self-learning module (SLM) was helpful prior to clinic.
It was useful to have access to training, resources and information in Canvas prior to clinic.
Did you feel that the training session was helpful in your preparation for participating in this clinic?
Please share your thoughts on how we can improve the training session and materials.
Please indicate your level of agreement with the following statement: You were adequately informed and prepared prior to this clinic.
How could we have helped you be better informed and prepared?
Are there any aspects of the clinic that you would like to see incorporated into the training to help you better prepare for the experience?
In addition to interviewing SMEs for this project, I was able to attend the face-to-face training session and clinic to observe the students and faculty in action.
Based on my observations at clinic, a new job aid was created to help students track arrival and status of patients waiting for check-in and triage. A resources module was also created in Canvas for admins to upload medical equipment manuals and job aids.
Out of 80+ faculty and students who attended the Apopka Clinic on June 28, 2018, 14 students and 6 faculty or volunteers completed the surveys. Some of the identified updates to the performance support site include lessons or modules on interprofessional skills for all students, more resources for medical Spanish and Creole, and prescription writing for MD students.
